Forte Residential, Inc. provides in-home support for individuals receiving services through the Medicaid Home and Community Based Waiver System. Currently, Forte Residential Inc is certified to provide services through the following Medicaid Waivers: Developmental Disabilities Services Waiver (DD Waiver), Support Services Waiver (SS Waiver), Autism Waiver and the Aged and Disabled Waiver (A&D Waiver).

Services provided through these waivers include  Residential Habilitation and Support, Day Service, Family and Caregiver Training, Respite services, Homemaker services, Attendant Care and Transportation as allowed by each respective waiver.

Forte Residential Inc provides Caregiver Support Services funded through State Line dollars. Recipients of this service are usually individuals that are on "waiting lists" for Waiver Services, and are contacted by the BDDS office if they are eligible for the service.

Forte Residential Inc. has also recently been approved to provide services through CHOICE funding (as of February 2009).

Transportation and residential modification needs are assessed and addressed by our creative and innovative staff. We work with each individual and their unique situation to pursue a barrier-free, fulfilling, safe, productive, and enjoyable life.

Forte Residential, Inc is currently approved to provide waiver services in 26 northern Indiana counties. These are: Allen, Boone, Cass, Clinton, DeKalb, Elkhart, Fulton, Grant, Hamilton, Howard, Huntington, Kosciusko, Lagrange, LaPorte, Madison, Marion, Marshall, Miami, Noble, Porter, Pulaski, Saint Joseph, Starke, Tipton, Wabash and Whitley Counties.
