Forte Residential provides in-home support for individuals receiving services through the Medicaid Home and Community Based Waiver System (HCBS). Currently, Forte Residential  provides services through the following Medicaid Waivers: Developmental Disabilities Services Waiver (DD Waiver), Support Services Waiver (SS Waiver) and Autism Waiver.

Services provided through these waivers include  Residential Habilitation and Support, Community Habilitation, Respite services, Family and Caregiver Training and Transportation as allowed by each respective waiver.

Forte Residential is a licensed Personal Services Agency, providing services through the Aged and Disabled Waiver (A&D Waiver). Forte provides Attendant Care and Homemaker services through the A&D Waiver.

Forte Residential provides Caregiver Support Services funded through State Line dollars. Recipients of this service are usually individuals that are not receiving services and are on "waiting lists" for Medicaid Waiver Services. Contact your local BDDS office for further information on this service.

Forte Residential is approved to provide services through CHOICE funding. CHOICE services are overseen by your local Area Agency on Aging office.

Forte Residential is currently approved to provide waiver services in 30 northern Indiana counties. These are: Allen, Boone, Cass, Clinton, DeKalb, Delaware, Elkhart, Fulton, Grant, Hamilton, Hancock, Hendricks, Howard, Huntington, Kosciusko, Lagrange, LaPorte, Madison, Marion, Marshall, Miami, Noble, Porter, Pulaski, Saint Joseph, Starke, Steuben, Tipton, Wabash and Whitley Counties.
