Forte Home Health Care, Inc currently works with Medicaid to provide:

Home Health Aide Services

Bathing, showers or bed bath
Oral care
Hair care
Dressing
Mobility assistance
Prepare, serve and assist with eating and meals
Activities of daily living assistance
Other tasks as allowed by law

Skilled Nursing Services

Skilled Nursing Services are available when the home setting requires specialized skills such as:

Comprehensive nursing assessments and reassessments
Medication monitoring and management
Supervision of Home Health Aides
Skilled care as ordered by the health-care provider

Forte Home Health Care Service Area

Forte Home Health Care, Inc. is currently approved to provide home health services in 23 Indiana counties. These are: Adams, Allen, Cass, DeKalb, Delaware, Elkhart, Fulton, Grant, Jasper, Howard, Huntington, Kosciusko, LaGrange, Marshall, Miami, Noble, Pulaski, Saint Joseph, Steuben, Tipton, Wabash, Wells, and Whitley Counties.

The mission of Forte Residential is to assist and support persons who are developmentally disabled or otherwise physically challenged through family driven initiatives to manage their own affairs in their own homes.
