Forte Residential History
Forte Residential, Inc. began as an entity in 1992 to provide residential services to a single individual, the son of the founders. In 1995, Forte expanded to provide family driven community based day services for the same individual. The benefit of “control in the hands of the family” was evidenced through the program’s success. In response to other families’ inquiries, the grass roots initiative began to expand. Forte now serves over 200 Indiana residents from Indianapolis north to the Michigan state line under the Community Integration and Habilitation (CIH) and Family Supports Waiver (FSW) waivers. Forte Residential, Inc. is also a licensed Personal Services Agency (PSA).

We are constantly amazed by the remarkable parents, siblings or close relatives who invest 100% of their lives to encourage, and guide their dependent loved one to reach their full potential. When end of life issues arise, extraordinary care that is put forth to assure their loved ones complete their lives with dignity and grace. The most valuable asset of an individual with challenging needs is the dedicated family member. Forte, a family centered agency from the start, wants to support the devoted family member in their quest to remain actively involved in the provision of home and community Medicaid Waiver services.

Forte has implemented technology to assure compliance to the standards, reduction of documentation errors and simplification of the record keeping process so staff can spend more time providing one on one personalized care for your loved one.
Forte Residential is accredited by The Council on Quality and Leadership (CQL).
Employee ownership positions qualified employees (individuals working 1,000 hours per year or more, approximately 20 hours per week) into a position typically reserved for a select few, one of ownership! Qualified employees (QE) share in the profits and financial risks of the company. Each QE has a mixture of cash and company stock in a tax deferred savings account. At the end of each year the company is valued by a third party and the value of their stock holdings is adjusted accordingly. Each QE is rewarded for their good performance and the overall success of the company. This encourages good business practices, long term employment, and company loyalty. The philosophy and the mission of the company remain intact for future generations as do the key employees. Such a culture of excellence should result in better services to your loved one.
Home Health Care Services
In 2012 Forte Home Health Care, Inc. was opened for business in response to requests of families in need of both Home Health Care (HHC) Prior Authorization (PA) and Medicaid Waiver services. It was decided that the request was consistent with the existing philosophy and mission of the company and that families deserve to have continuity of care. By cross training existing staff to perform both services, families have fewer employees coming into their home, interaction with only one company. Schedules are coordinated with greater ease, information moves more freely, documentation methods are consistent and the philosophy and mission remain the same. This has proven to be a good decision for these families.
The services of Forte Residential, Inc. are founded on the belief that individuals with disabilities are valued members of society and valuable to God. Every individual, regardless of ability or disability, should live with dignity, a positive social role, inclusion into the community, self direction, and a feeling of self worth. Individuals who require support are of great worth and should be held in high esteem. That God in the person of Jesus Christ and through the Bible has established moral and ethical truth, the basis for the conduct of all people.
Forte Residential, Inc. seeks to provide the highest standards of quality services to individuals served by elevating the consumer’s legal representative to the role of Family Advocate, a role with a voice in the administration of active care. The consumer and parent / legal guardian will be given as much control over significant issues related to their care as allowed by existing law and Forte Residential, Inc. deems appropriate to assure the safety, well-being, and benefit of the consumer.
The mission of Forte Residential is to assist and support persons who are developmentally disabled or otherwise physically challenged through family driven initiatives to manage their own affairs in their own homes.
